The HSC Foundation is dedicated to improving access to services for individuals who face social and health care barriers due to disability, chronic illness, or other circumstances that present unique needs.
The HSC Foundation accepts proposals by invitation only. However, we welcome information on programs relating to youth with disabilities transitioning to adulthood, as well as support, education, and training programs for families and caregivers of individuals with disabilities and chronic illness.

The HSC Foundation has launched a Youth Transitions Initiative, designed to become a long-range signature project to assist young people with disabilities and chronic illness in the Washington metropolitan area and beyond to move from school to adulthood to the world of work.

The HSC Foundation is committed to supporting families and other individuals who are providing care for infants, youth, and adults with chronic illness, disabilities, at-risk for disabilities, or who have unique health care needs.

The HSC Foundation helps targeted communities within the Washington Metropolitan Area to "live healthy" by supporting programs that promote healthy homes, healthy individuals, and healthy communities.

The HSC Foundation operates two awards programs—one designed to enrich the lives of at-risk youth, including young adults with disabilities, in the Washington-Baltimore-Richmond area, and one that recognizes a young person who has distinguished themselves through their professional or personal contributions in the area of disability.
